A patient-centered health journal that converts brief daily check-ins and imported measurements into clinician-ready visit summaries.

Medium opportunity (63%)
People managing chronic illness, chemotherapy, or complex medication schedules need longitudinal records to understand flare-ups and communicate with clinicians. Existing trackers can demand dozens of repetitive entries while providing weak interpretation, incomplete health-data imports, and reports that do not clearly surface medication changes, triggers, injection sites, or symptom timelines. The burden causes patients to abandon tracking precisely when continuity matters most.
Build a focused health journal that starts with a personalized tracking protocol for the patient's condition and asks only a few adaptive questions each day. It should import available measurements from Apple Health and common wearable sources, record medications and injection sites, and generate a concise appointment packet showing symptom changes, possible associations, adherence, and an editable timeline. The product should describe associations without presenting them as diagnoses or treatment recommendations.
Patients increasingly have useful measurements scattered across wearables, health platforms, medication logs, and clinical portals, but still need to assemble their own story for appointments. The signals show sustained use when tracking produces credible evidence for clinicians and abandonment when manual entry outweighs the value of the resulting analysis.

Useful App I’ve enjoyed using this app and knowing if I am getting enough rest and sleep. Sleep Tracker has been very helpful in giving me the data I need to keep an eye on that.
Customizable I do love that it’s very customizable. Maybe I haven’t figured out the best way to use it yet (it’s been two months), but I hoped it’d like the graphs and info feedback better. I wish more things showed up as trends on the dot graphs. The other information and feed back it gives hasn’t been terribly helpful or easy to use. It also is a bit overwhelming to fill out every day - I know that’s more the fact that I have so many symptoms and factors though. I think it’s good to track so I do it anyway but I’m starting to really dread it and think the info it gives/the way it’s presented back may not be worth it.
The Health Tool Of A Lifetime This app is amazing for quickly tracking your health symptoms and offers so many more options such as medication tracking. I love how customizable this app is to each individual. The only suggestion I would make so far is to allow users to add in their own medication list as some medications are not available to document. GET THIS APP!!! You do not have to pay a dime to use it for free and the functions are still extremely useful!
